Our office is in the Dunn Loring/Merrifield area with access to Metrorail, shops, and restaurants. A hybrid work model is available. Care Connection for Children is part of Inova’s Community Health Division and is a partnership with the Virginia Department of Health. As a Social Worker Case Manager I, you will use your communication skills, knowledge, and compassion to empower families. You will provide/evaluate biopsychosocial impact on patients' plans of care. To help achieve our mission, you will evaluate the ability of patients to progress throughout the continuum of care. Working collaboratively in communication with healthcare providers, schools, and community programs is of vital importance. An understanding of post-acute and community resources is required. Your ability to provide coordination of services and act as a key liaison between patients, families and the interdisciplinary team is expected. A background in healthcare, community health, and/or community social services is a plus. 

Job Responsibilities:

• Assesses the psychosocial risk factors of patients/families through the evaluation of prior functional levels, appropriateness/adequacy of support systems, reactions to illnesses and the ability to cope.
• Intervenes with patients/families regarding emotional, social, and financial consequences of illness and/or disability.
• Collaborates with the interdisciplinary care team, patients, and families in the assessment/ coordination of ongoing care in the community.
• Serves as a resource person and provides support to families related to the needs of the child/youth.
• Works collaboratively in a team environment to achieve desired outcomes for assigned patients/families.

Requirements

• Education: Masters Degree in Social Work (MSW).
• Experience: One year of clinical care or clinical case management experience. Child health experience a plus.
• Certification: BLS through the American Heart Association upon start.
• Bilingual Spanish-speaking candidates strongly preferred.
